For your convenience, we are providing Patient Registration and Medical History Forms for you to download. Please fill them out as adequately as you can and bring them in at the time of your next visit.

The form requires Adobe Acrobat Reader to open and print. Get Adobe Reader, its FREE and fast to install. Please make sure to uncheck Adobe Yahoo! Toolbar and Adobe Photoshop® Album Starter Edition if you do NOT want them installed.

Emergency Hours
Emergency Hours are available for patients. Our schedule allows for daily emergencies if necessary.

Insurances/Finance - We are flexible and offer payment plans to meet your budget needs. We offer a discount for cash payment in advance of treatment and also accept Visa and Mastercard. We will gladly assist with the filing of insurance forms and paperwork. The billing office recently installed an insurance estimator that will determine your portion of the proposed treatment prior to scheduling to help you finance the procedure.
